adjust document structure #10

Update document structure to have one "test" adoc per Global Service.

A PDF render of this PR is attached for reviewing in context.

Note: in global-services-testing/sections/testing/global-cache.adoc, I've marked some areas as TODO cc @jmaxmarno

Note that initial GDC Functional tests are now included in this PR.
