Add a new documentation.odd to handle documentation needs #4

Duplicating the entire teisimple.odd just to add handling for <code> seems a bit wasteful, but I can't think of a better way at the moment. I recall there's a way in ODD to reference a different ODD, and then specify one's changes against that. But I forget the exact mechanism. Perhaps we can discuss this with the TEI-Simple folks in a few days..
