Add Topic option to item authoring #2

i.e. in CAD, let you categorize items by topic (drawing, file commands, etc.), so it's easier to view in the item list.

Goals: Let's put in an optional field where someone can pick a topic / module for an item. If they then tag the question with an learning outcome, it should "overwrite" the topic and infer the module name / folder from the outcome.

Questions should also be visually separated in the item screen somehow, i.e. tabs or folders

Jen will send objective hierarchy -- then we'll just do this the right way.

This should be address on the server-side code, commit f97fdeaacb9b8b12f31a34eec1396eef0b8d3f54
