how to make data use agreement? #4

hello, eveyone

I'm curious how I can get the data, so I leave a question. in official challenge site, "The following dataset is made available, upon request and after signing a data user agreement" but i don't know how to make data user agreement. Is there a set format for agreement?

ps First, I sent a data request email without data user agreement

Dear Olrun, Thank you for your interest in the challenge. Please fill in the Data Usage Agreement https://www.dropbox.com/s/nk1h1yq11hq3702/Data%20Usage%20Agreement_2021.doc?dl=1 (DUA) and send it back signed. Please update the highlighted parts of the agreement with the actual organization name and effective date. When we receive the signed agreement, we will provide you details on how you can download the training dataset.
