Ensure default credit card form field works in tev1 and tev2 #2277

I'm not 100% sure this currently works - and if it does work; we need to make sure it's best-of-class in tev2.

This would include some default opinionated styling, masking, JS to verify card type, etc. Some research should be done (via Baymard, other research/best practices) as to the best default form usage here. Look into what other platforms do here as well (Shopify, Woo, EDD, etc.)
