Consider removing the `.settings-fields` wrap element #37

@afercia and I have a different opinion, so let's further discuss this over time, hopefully with more input soon.

@afercia says that the .settings-fields div should be removed to prevent divitis, but I think it's necessary to be able to provide more precise styling. See https://github.com/wpaccessibility/settings-api-enhanced/pull/32 for more background.
