Avoid indexing or transferring PageRank

Comment mail pages are being indexed by search engines because of these links. I did a Google search for "site:domain.com comment_mail" and I have 17 results for "Add New Subscription..." pages. This increases if you have more posts in your site.

@IvanRF Thanks so much for the Pull Request—very sorry about the late reply here. Would you mind submitting this PR to the Comment Mail Pro repo? Comment Mail Lite is built from the Pro repo, so submitting your PR there will allow me to merge in those changes so that they get applied to both the Pro and Lite versions. :-)
