Unable to delete forms and its data from server

What steps will reproduce the problem?
1. Go to form list and click on "Delete" Form
2.

What is the expected output? What do you see instead?
The form and its data should be deleted.

Instead of it appears again.

What version of the product are you using? On what operating system?
ODK Aggregate v1.4.4

Please provide any additional information below.
The form which cannot be deleted their data are published in fusion table 
previously. But now when I open the "Published Data" tab and select those forms 
it shows error "Error: Problem persisting data or accessing data (Somehow DB 
entities for publisher got into problem state)".

Kindly help me how can I delete those form from my server.

Hi,

I am also facing the same problem.Even i changed the apis credentials.
The only option I thought to dump the database and create a new one.
