Rename migration docs #284

Description:

Currently, the MI migration documents follow the initial title pattern used when the documents were created since MI was released as a part of APIM.

For example: Upgrading from WSO2 API-M 4.0.0 to WSO2 API-M 4.2.0

However, since MI is a separate product, we can rename the files to follow the format below:

For example: Upgrading from WSO2 MI 4.0.0 to WSO2 MI 4.2.0
