Multiple "Change Password" modals appearing in certain pages

Description: The user password change modal produces multiple instances in specific pages. These pages include:

• Device Type Management

• Role Management

• Policy Management

• Certificate Management

Suggested Labels: Priority/Low

Affected Product Version: wso2iot-3.1.0-update11

OS, DB, other environment details and versions:
Tested on Ubuntu 16.04 LTS

Steps to reproduce: Go to any of the before-mentioned pages and select the change password item in the user dropdown. Then we have to close through 2 modals on top of each other instead of only 1 as on unaffected pages.
