**Form Creator: popup doesn't disappear #126

When you click on the "variable" link next to "value" it presents a pop-up but does not disappear when you click off it on the form. Also doesn't react to the escape key.

Fixed. The issue was a combination of markup that changed, misplacement of modal, and un-indentified id in the css and js.
