Additional options for form #238

On the same tab as "production ready" we need two more check boxes.

  1. Export to Public
  2. OAI-PMH Export

These boxes have been added in the linked_forms branch. I'm closing the issue, but the branch will remain until issues #236 and #235 are closed.
