Add forkme on github #7

Several issues on the cargo repository are centered around the need for links to the repository from this website. This pull request adds a "fork me on github" link.

Unfortunately, I ran out of time to get a modern version of Ruby running that was compatible with middleman. You should take a peek to make sure this looks right. Sorry for the inconvenience!

@brycefisher For future reference, you can download https://github.com/tokaido/tokaidoapp/releases, open the .app, and click "Open in Terminal".
