Use the newer ppx version of the cstruct library
Update to PPX-based NBD library

The other part of the ppx-ely branch was ported to master in this PR: https://github.com/xapi-project/vhd-tool/pull/35

I managed to build it successfully in a ppx-ely-base planex-buildenv container, I just had to remove all the generated build files.

The opam file also needs to be updated to require the newer library versions, but we can do that in a separate PR - I don't know right now what minimum versions we require, and the opam file of this repo is not that important, as we are not using it as an OPAM library/package.
