First attempt to fill the gaps in this spec #439

I have tried to fill some gaps in this specification. If this is not complete nonsense I would suggest to merge this and have a further discussion on this base. As the original specs had no explanation of what will appear on the result port I spelled out what XMLCalabash and MorganaXProc do for 1.0. Not sure that should be the last word on this topic. A rendering is here.
