Attempt to resolve ambiguities in p:cast-content-type #619

  1. Clarified that supporting additional XML-to-JSON conversions is implementation defined.
  2. Documented c:param-set and c:param. The declarations were already in the schema, luckily.
  3. I believe the prose is now prescriptive regarding the c:param-set and c:param elements.
  4. I think c:data is defined.

Note that the description of c:param is broader here than it was in 1.0. But we made those changes in the schema long ago and I assume we did it on purpose.
