What steps will reproduce the problem?
line 99:
        $msgerror .= "Please provid a Username.<br>\n";
should be 
        $msgerror .= "Please provide a Username.<br>\n";

What version of the product are you using? On what operating system?

Thanks for reporting the spelling mistake. This is fixed on revision 1306.
