404 Not Found - Fax Server

What steps will reproduce the problem?
1. Login as User
2. Click on "Apps" -> "Fax Server"

What is the expected output? What do you see instead?
Fax Management is expected, 404 Error appears.

What version of the product are you using? On what operating system?
FusionPBX "1876" on Ubuntu Precise 12.04 LTS.

Please provide any additional information below.
Link to http://url/app/fax/fax.php works instead of 
http://url/app/fax/v_fax.php.

In a changed a file name a couple weeks ago v_fax.php is now fax.php.

You upgraded recently and did not update the menu. To update the menu. Go to 
the menu manager and press 'restore default'.
