provision v_line_displayname customization

On phone provision the v_linex_displayname is the effective caller id name, but 
there are a lot of situations where you don't use this field, and by other side 
this is not the best information that you need on the phone, something like the 
extension followed by the effective caller id name could me more usefull. This 
could be even better if I could have that as a default, but this default could 
be overrided by a domain variable where is possible to define the template 
string for the v_line_displayname.
