Add the account code field in dialplan_inbound_add.php

When create gateways for outbound call we can assign account code, but in the 
dialplan_inbound_add.php to add incount routes there is no field to add account 
code, we can manually add set account code step after submitting the dial plan, 
but it would be nice if we have an option to set it when creating the inbound 
route.

PS: I can make the changes myself, can I commit it to the repository? Would be 
glad to be able to help and contribute.

The inbound route could have accountcode as an option. Probably best to wrap a 
permission around it so that the permission can be enabled and people that want 
to use it would have the option and those that don't won't have an additional
