PUK Screen don't work if you insert wrong PIN #145

What steps will reproduce the problem?
1.Write wrong PIN 3 times
2.You get only a screen with a text but no field where you can insert the 
PUK/Super PIN

What is the expected output? What do you see instead?

What version of the product are you using?
1.      the right 0317 thing
2.  system-gb-20110529-BETA.zip 
3.  boot-gb234-110509-rndis-236mb.zip 

On what operating system?

Do you have logcat or dmesg ?
 (adb logcat -d > logcat.txt, adb shell dmesg > dmesg.txt)

What version of the *Official LG* Firmware were before using Custom
firmware ?
(V20G, V20D, V15x, V16x etc. )
V10E
