Create 'Open with Permission' placeholder image

Story Currently, the 'Yale Community Only' image is being used as a placeholder in Blacklight for objects that have a Visibility of Open with Permission. We need to create a new placeholder image that accurately reflects the Open with Permission Visibility.

Image should include the following language:

ACCESS AVAILABLE by request only due to collection restrictions. Please log in to request access.

Acceptance

[x] New image file created for Open with Permission placeholder
[x] Image includes text as described above in Story
[x] Ticket created to add image to Blacklight feature branch -- created #2804

Tracy has made it 500x500 so it will scale for responsive design -- the width and height in the code will need to be set to 250 x 250.

Question: Are quicksearch thumbnails given a set width and height? Is making this larger than 250 x 250 going to break quicksearch?
