Add Approver Notes to Permission Request Data Table #2846

Story Our main OwP stakeholder has requested that Approver Notes be added to the Permission Request Data Table that is available in the Management App on the/management/permission_requests page.

This will allow Approvers to review all Approver Notes associated with a parent OID or a user, depending on how they filter they table.

Acceptance

[x] Approver Notes added to data table
[x] Column added after Request Status and before Date Approved

Do we want to add any formatting to the datatable field? How long do we expect approver notes to be?

Deployed to Test and Demo with release v2.69.9

I can't image these will be paragraphs long. We can always re-tool later if needed, but for MVP I think this is fine. Looks good on Demo MGMT, can see notes in table and they export in spreadsheets :)

I think we can close this since we don't have any requests in UAT :)
