Add an additional outermost level of headings. This will help when the Processes chapter is broken out into six individual chapters and Information Models is broken out into four. Chapter numbers are not affected.
As part of this, existing headings are all incremented by one level.
This also lets us simplify the auto-numbering a bit more. Appendix letters are no longer a special case.
