nested sections #22

When file have multiple nested sections, they are not properly nested in TOC.
To be investigated further.

The problem is that TeX allows only:

section
subsection
subsubsection

While in FB2 we can have arbitraty level of nesting.
Perhaps we can first 3 for first 3 levels and then stick to subsubsection...

Here is an hierarchy I plan to use to number nested sections:

part
chapter
section
subsection
subsubsection
paragraph
subparagraph
