miscellaneous small changes #286

Minor/cosmetic changes as I've been testing usher-sampled-server:

added some details to the server option descriptions
added outdir arg to client-example for testing simultaneous queries
accept '*' as an alternate allele in VCF (like N; warnings were clogging stderr log)
gitignore all build*/ directories
