symptom/sign #17

- OGMS: symptom is observed by patient - sign is observed by physician:
seems to be the only difference between the 2 terms, and I am not sure this
is very intuitive. Do we need both? Should we define a new term covering both?

Do we want to link symptom to the symptom ontology
