• [x] Does donate now take you to azpeople?
• [x] Please take out middle name requirement on Applicant Information
• [x] on assistance request page please add a line stating that they will need to provide attachments for the hardship they are claiming under at the bottom of the page
• [x] There should be a space between at and least on the assistance request page
• [x] attachments page not letting me add attachments
• [x] can we say that we will accept the following attachments?
• [x] the review and submission page makes me print my name and then sign. If I put my name in the print name first it makes me retype it after the I click accept.

Donate now does take you to azpeople, you just have to sign in again first.
