As AAF admins I want to know what other assistance is my applicant receiving So that I can better evaluate the amount of help needed

[x] Modify layout to use Bobby's design
[x] Remove the previous button
[x] Add a save for later button to allow user to exit the application
[x] Build in interaction with data provider service
[x] Change the title to ASSISTANCE REQUESTED AND/OR RECEIVED (not all cap)
[x] Change the title on the side bar to match
