[ ] change file name from cpp to hpp
[ ] create template, change namespace according to input file and private to protected
[ ] update openrave installation instruction and create a new Moveit! tutorial for kinetic following the original one for indigo

I don't think we need this, given that we can directly use descartes's ikfast moveit adapter as a plugin.
