Provide index.js upon installing via NPM #5

index.js is not being provided when installing this package via 'npm install' or 'ember install:addon'.

This breaks out compatibilitiy with the latest ember-cli version: https://github.com/ember-cli/ember-cli/releases/tag/0.2.0
