Update issue templates

Added basic template for the bug report.

I tend to find these templates of limited utility. I wonder what @ltratt thinks?

I'm not a fan I'm afraid, at least not for small projects. For very popular projects they're perhaps useful as a rate limiter of sorts, but for the small scale we're currently working at, they don't add value.
