Change of copyright notice

The license of the whole code is GPL 3 and is not going to change anyhow. 

However there are copyright notices in the beginning of each file (and also in 
stdout), like
  Copyright (C) 2006-2008 University of Amsterdam
  Copyright (C) 2009,2010 Institute of Chemical Kinetics and Combustion & University of Amsterdam
While these notices are supposed to note the history of file changes and 
different contributions, it becomes more and more complicated to properly 
maintain them. Moreover, these notices are not really necessary since a 
detailed history of contributions can be built from the commit logs.

So I propose to replace all these notes by something general like 
  Copyright (C) 2006-2010 ADDA contributors

Similar problem is with field $Author: ... $, which is automatically filled by 
Subversion with login name of the last committer. This may be misleading since 
the last committer may have done minor changes compared to the original author. 
Since this information is better assessed through commit logs, I propose to 
remove this field.

Of course, this general header should apply neither to contributed code in the 
'misc' folder nor to the open-source code for routines included in the main 
trunk. The latter include, e.g. Mersenne twister (random generator) and 
Temperton FFT routines.
