I also had this same problem with the death sensor just plain not working. Substituting it with a health sensor worked fine but the death sensor is DEFINETLY broken in some way. Short Description
The Death Sensor doesn't send out it's code when the user dies.
Reproduction
Add a Death sensor with the send code of "1"
Add any program and deactivate it but give it the Activate code of "1"
Die.
Program doesn't activate.

Was this done on a locally hosted or non-Yogstation hosted server?

Unsure if this is to do with the send_code() proc but using a remote control and sending the "1" code activates the program like it would if the death sensor were to work.
