1 - Update Booking Process

Please update the booking process by:

• removing the travel charge field from signup process, and from "My Account" menu. We don't need this field in these places anymore.

• adding a travel charge button on the "To Confirm" under list for MUA on the booking item (see sketch file). When user selects "travel charge" it will slide to the "Enter amount" text input field. This should now be the travel charge field on the backend.

• The user can enter any amount of money in the travel charge field and select Confirm to finish the process. The rest of the process should stay the same except that the customer user who requested the appointment will see an updated total charge for services. There should now be an additional line item for travel charge.
