screen,

  1. Select the ADD PATIENT INFORMATION option
  2. Fill out all fields First Name: “1Juan” Middle Initial: “2” Last Name: “Dela Cruz3” Sex: “M” City: “Manila” Birthday: “01/20/1994”
  3. Click the ADD PATIENT button
