Rework rejected maps list #122

add ´challenge.is_rejected´ and challenge.rejection_reason
change the table to:
- list all maps that are rejected themselves or have rejected challenges
- group all entries for the same campaign
- ALSO group all entries for the same map 😭

Might be better to:

split the table into 2 tables for rejected maps and rejected challenges
In the rejected maps table, group maps by same campaign
In the rejected challenges table, group challenges by same map

There should also be a video link for each rejected challenge.
