Link App to its swarms #146

For each App, it would be nice to know which swarms are available for that App (even if they have no deployed procs). Create a series of links in the /app/{id}/ to link to those swarms.

In general, I would like every object in VR UI to be a link to its specific page. E.g. on the app page, have a link to the coresponding buildpack page. In the buildpack page, have a link to the apps that uses it. In the swarm page have links to ingredients, and viceversa. Etc.

It would make navigating through objects much easier.
