MT: start threads on demand (not immediately) #22

Changes a few things in the MT context:

takes a range in addition to a fixed size (e.g. ..4, 2..4 or 4..4);
no longer starts all threads immediately;
no longer waits for threads to be started to continue (not needed);
on wakeup, eventually checks if a new thread should be started.

Implements the first half of #21
