Plugin should allow files to be added to review record from file context menu. #85

This is an enhancement request to allow the file context menu to add the 
selected file to the review record. I have made the change to the version 
of the plugin that my team uses and we find this to be extreamly helpful 
for keeping the reviews tied to the files. 

Also find in the patch an addition to the ReviewAdditionActionDelegate 
class that adds the file to the review as it's adding the item. This way 
as you add issues you are capturing the file into the file list.

By the way, is this patch meant for the version of Jupiter that is currently on 
the
trunk of the SVN repository?
