fix out of bound accesses #18

I found some out-of-bound errors. I'm not sure this fixes (clamping values) are good, I hope it helps you correct the code.

@yukiteruono how about this and three other PRs I've opened? I'd like to know if will merge them or not (or if you're preparing better fixes for them)
