I have the same issue. FireTV and latest version of SmartYouTubeTV, however it DOES happen with the TV speakers as well as the Bluetooth.
That's interesting. I can neither confirm not deny since I've never used the FireTV without Headphones.
Update / Workaround I've been using the 4K Alt (Exoplayer + XWalk) mode for a while. And there were no issues of this sort.

This is a rather weird bug that only happens with SmartYouTubeTV and only when using Bluetooth Headphones.
Description: The sound that is emitted is hard to describe, it's a rather short noise (<1sec) and really unpleasant. It's not always the same, but about the same duration. It's recurring every minute.
Where and when does it happen: Anywhere in the App. While the app is in foreground. It doesn't matter if a video is playing or not. It seems to happen in the 4K Mode. I haven't used any "Alt" modes. In 1080p it happens rarely, mostly I'm able to use the app for days without spotting the issue (on 1080p mode). I can easily reproduce it with 4K anytime.

I couldn't find any related issue and I feel rather lost right now and can't find a link between BT Audio and the functionality of the App.
