Add SKIP annotation #99

An SQL file sometimes includes statements that are not to be sent to the 
database. For example SET DEFINE OFF which is an option of SQLPlus.

We need a way to ignore these statements:

--* SKIP
SET DEFINE OFF;
--* /SKIP
