Does not work on Galaxy Nexus 4.1.1 #13

Running Permissions.apk doesn't seem to work on a GSM Galaxy Nexus running the 
latest OTA'd 4.1.1 firmware.

I see the splash screen, and then the "Do or Do not there is no try" screen 
with no list of apps.

Superuser does not report an su request from Permissions unless I use the 
three-dot ActionBar thingy to request a reboot.
