thinkbayes DocBook: exercises should be labelled as exercises #14

For exercises I am currently generating this tag:

<example id="a0000001065" role="exercise">

In the PDF, this should be rendered as Exercise x.x.

But at the moment it is Example x.x

Should I generate different DocBook?

As for issue #14, we actually don't have handling in the stylesheets for
an "exercise" role. But I should be able to add handling for it with a
stylesheet customization. I'll take a look at it next week and get back
to you.
