This describes a failure condition in which a heading level is skipped. Even though the second heading visually appears to be a third-level heading, PDF/UA-1 prohibits skipping heading levels, to ensure a navigable document. In this case the tags use H1 followed by H3, so the heading level H2 is skipped.
