Fixes for compatibility with Strava UI update #12

Resolves #11.
Strava changed the url and UI of the heatmap the other day and this extension stopped working as a result. The fixes consist of:

Adjusting the url match pattern so the extension will be active on the new page
CSS updates to work with the new UI
Changes to how the currently selected sport and color parameters are accessed. Instead of getting them from DOM, the are now pulled from the url query string.
