Added Copyright template - COPYRIGHT.md #44

As it was agreed a long time ago .... we need COPYRIGHT.md file and then in headers we are going to have reference to that file.

As I'm updating zend-coding-standard we need template in one common place.

I've been thinking about this some more, and I'm starting to question having a separate file for the copyright, considering that the license file already has copyright information.

Shall we consolidate?
