Follow up to PR #40

updated .gitattributes and .gitignore
moved all support files into docs directory, updated and added missing templates
updated composer.json
updated Travis CI config
- removed composer self-update
- removed allow_failures on PHP 7.2
- moved cs-check and coverage test to run on PHP 7.1 with locked deps
- updated legacy deps
