Prepare documentation for publication #105

This patch prepares the documentation for publication:

Renamed files to remove zend.input-filter. prefix.
Checked documentation for code blocks (Fixes #89).
Edited all documentation for content and formatting (Fixes #90).
Converted from bookdown to mkdocs.
Added documentation build automation.

In order to accomplish the latter, switched to the lowest/locked/latest CI strategy.
